SPECIMEN REQUIREMENTS
Instructions Call Customer Service at 405-307-1100 for locations and hours of operation. 1. Wash hands and genital area with soap. 2. Rinse thoroughly to remove all soap. Soap will kill sperm. 3. Open sterile, glass or plastic container. 4. Masturbate to ejaculation. Other methods of collection may compromise specimen and patient may be required to provide another specimen. Note: 1. Abstain from ejaculation for a minimum of 2 days but not longer than 7 days before collecting the specimen. 2. Do not use lubricants as many lubricants can kill sperm. 3. Avoid vaginal and oral secretions, which will contaminate your specimen. 5. Collect entire ejaculation into sterile container. Do not touch inside of container. 6. Tightly secure cap to container. Keep container upright. 7. Protect specimen from temperature extremes. Note: 1. Keep specimen away from direct light. 2. Maintain specimen as close to body temperature as possible. 3. Do not heat specimen above body temperature, as this will kill sperm. 4. Do not allow specimen to drop below 70 deg C, as cold temperature will adversely affect sperm. 5. Label specimen with patient name (first and last) or other unique identifier, patient hospital identification number or Social Security number, date and time of collection, collector initials, and test(s) being ordered. No not label lid.
